Based on all such evidence, we hypothesize that HCQ and ivermectin could act in a consequential and synergistic manner. Indeed, HCQ would behave as a first-level barrier by inhibiting the entry of the virus into the host cell, while ivermectin could reduce viral replication if the virus did get in, strengthening HCQ antiviral effects. HCQ is cheap to produce and safe if monitored properly. Ivermectin seems to be safe and well tolerated, with no serious drug-related adverse events.
Moreover, the 2 drugs do not seem to have a between-drug interaction. However, no in vitro or in vivo studies have been conducted on the combined effect of HCQ and ivermectin on SARS-CoV-2 infection.

Conclusion: Data from three outpatient COVID-19 trials demonstrated that gastrointestinal side effects were common but mild with the use of hydroxychloroquine, while serious side effects were rare. No deaths occurred related to hydroxychloroquine. Randomized clinical trials can safely investigate whether hydroxychloroquine is efficacious for COVID-19.

The Saudi national study of some 8000 outpatients showed 5-fold reduction in mortality of HCQ+zinc vs zinc alone. Outpatient studies of HCQ by itself tend to show 2-fold hospitalization/mortality reduction. Evidence that HCQ works, but HCQ+zinc works appreciably better.
Most notably HCQ treatment resulted in a higher proportion of negative nasal swabs at day 3 of 6 after treatment was started.
NOTE: HCQ or chloroquine is chemically identical to the active ingredient that was found to cure malaria, natural quinine that is made from Cinchona tree bark.
